Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current dental insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Medical History Form

Please note: If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Acrobat® Reader'. This will take you to Adobe's website and enable you to download the Acrobat program to your computer.

Exams/ Consultations

Typically, our New Patient exams will last approximately one hour. Dr. Warren will determine what x-rays you will require after learning of your complaint.

Emergency Exams: Most emergency appointments will require two to four x-rays and Dr. Warren will diagnose and deliver a treatment plan that will address your dental problem. After the clinical exam, a member of our team will discuss the financial portion of the needed procedure and will make the appropriate payment arrangements at this time.

Comprehensive Exams: A comprehensive exam will require a complete series of four to eighteen x-rays. After having the x-rays, Dr. Warren will evaluate your dental needs and present you will treatment options that will restore your dental problems and satisfy your concerns for your mouth. Most comprehensive exams are often times complicated and require a comprehensive approach to fulfill the wants of a patient. If there are multiple treatment options or if there are a large amount of restorative concerns, Dr. Warren will schedule additional time to personally visit with you regarding your dental treatment. Dr. Warren will make every effort to assure that you understand the condition of your dental health, no matter the severity. After Dr. Warren has evaluated your restorative needs, we discuss any follow-up treatment with you in detail, including the cost for your particular case.

Regular Appointments

Regular checkup appointments typically take 30 to 60 minutes. Patients are seen by appointment only. We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately.

Insurance

We will be more than happy to process any of your insurance forms for you. We understand that insurance can be difficult to understand and we have a dedicated team member to address just these concerns. We will file your dental claim electronically so that your insurance payment can be expedited and your services not delayed. Any amount that the insurance does not cover is the patient’s responsibility.
